Step Two: Enter Prescription Info

If you are a U.S. customer, your prescription information is required in order to ship your order. There are two options for verifying your prescription information:

1. If you have a copy of your prescription from your eye doctor, you can fax or email it to us to speed up your order. Please fax to 800.929.3709 or email .(JavaScript must be enabled to view this email address). Make sure to include your order number when sending prescription information. 
2. If you are unable to send us your prescription, you must fill out a prescription request form so that we can verify your prescription with your doctor.
